Application to Audit Course

Form will be available on the 1st day of each term

Complete this form to request approval to audit a course. Steps to audit a course:

  1. Use Course Lookup and check to see if the course is still available.
  2. Obtain email approval from the instructor via their UTD email. Be sure to include the term, course prefix, number and section of the class that you are requesting to audit.
    1. Remember that some courses cannot be audited.
  3. Submit the completed Application and Instructor’s email approval to records@utdallas.edu. If a current student the submission should come from your UTD email account. If not a current student, include a copy of your ID for verification of address and date of birth.
    1. Deadline to submit the application form is the full-term census day for the semester. Census day can be found in the Comet Calendar.
    2. If over 65 years of age, a government issued ID showing date of birth will be required. If under 22 years of age and not a current student, vaccination documentation is required.
    3. If you plan to audit a graduate course, transcript proof of undergraduate degree is required.
  4. Once you receive email confirmation of the registration, visit the Bursar’s website to complete online payment of the audit fee.

Change of Admissions Application Request

Applicants who have not been admitted to UT Dallas can use this form to request to change the program/plan/term on your application. A student can only change an application term forward or back one long semester (fall or spring). The request must be filed by Census Day of the current application term. To submit a Change of Admissions Application request, log in to Galaxy, then submit an online Change of Application Request web form under the Admission Forms folder.

Enrollment Certification – National Student Clearinghouse

Current students fall 2011 and after, may connect with the National Student Clearinghouse using the 10-digit UTD-ID and obtain copies of documents confirming enrollment at UT Dallas for personal records. Students needing enrollment certifications for auto insurance, scholarships, health insurance and with GPA information may not use the National Student Clearinghouse service. Please follow the Enrollment Certification – Orion Enrollment Verification option.

Meningococcal Meningitis Vaccination Requirement Form

Per State legislation, all entering Texas college students must receive a vaccination or booster (if the vaccination is five years old) against bacterial meningitis before enrollment. Complete the form and submit the necessary documents through the Galaxy portal no later than 10 calendar days before the semester begins. See the Vaccination webpage under Legislative Policies for instructions on how to upload the documents.  Students will be expected to submit proof 2 weeks prior to their orientation date for review and processing. The last day to receive the vaccination in order to attend the Spring 2025 term vaccination is January 11, 2025.

Meningococcal Meningitis Vaccination Verification Request

Current, alumni and former students who submitted their proof of immunization prior to enrollment can obtain proof that they met the requirement.  We do not provide copies of vaccination records.  However, we can provide a letter verifying the date you received the Meningitis vaccine. 

Name Change Request

This form must be accompanied with proof of change, i.e. valid drivers license, passport, marriage certificate or court order.

Petition for Graduate Deferment of Admission

A statement of reason and any supporting materials the applicant believes to be relevant and wants the Academic Program to consider should be submitted with this form. To submit a Petition for Graduate Deferment of Admission, log in to Galaxy, then submit an online Graduate Deferment of Admission web form under the Admission Forms folder.

Petition for Undergraduate Deferment of Admission

The deferment is only valid for the semester of the student’s original application. Admission can only be deferred for one semester unless the student is a member of the Transfer Comet Connection program. Deferment request must be filed by Census Day of the original application term. To submit a Petition for Undergraduate Deferment of Admission, log in to Galaxy, then submit an online Undergraduate Deferment of Admission web form under the Admission Forms folder.

Petition to Use National Test Scores as Transfer Credit

For undergraduate students only. Starting Fall 2023 students are able to petition for their own test credit. Please review the undergraduate dean’s website for details regarding test credit that applies.

A student, who has been placed on suspension, must complete and submit for approval the Undergraduate Academic Suspension Readmission Petition form along with the admission application through ApplyTexas.  Please be sure to check the academic calendar for deadlines.  Readmission is not guaranteed.

 

Residency Verification Letter

Core Residency Questionnaire

Core Residency Questionnaire- The Texas Administrative Code, Rule §21.25 requires each student applying to enroll at an institution to respond to a set of core residency questions for the purpose of determining the student’s eligibility for classification as a Texas resident for tuition purposes. The University requires you to submit a Core Residency Questionnaire and supporting documents when seeking reclassification based on additional or changed information. SSN Change Request

Texas Tuition Rebate

Using NetID and Password, Sign in to Open the Texas Tuition Rebate Application form.  Follow instructions to complete, and Click Submit.

Application for qualified undergraduates to receive up to a $1,000 tuition rebate. Submit after applying to graduate for a first bachelor’s degree. If you receive an unable to load form error, please clear your browser cache and cookies then try again. For any other issues with completing this form, please email records@utdallas.edu.
